Horizon BCBS NJ Coverage for Detox & Rehab
Horizon is the largest health insurer in New Jersey. Most Horizon plans cover medical detox and residential treatment as in-network benefits, subject to medical necessity review.
What’s typically covered
- Medically necessary detox (inpatient and ambulatory)
- Residential rehabilitation (14–28 days initial authorization)
- Partial hospitalization and intensive outpatient
- MAT: buprenorphine, naltrexone, methadone
Prior authorization
Emergent detox admissions do not require prior authorization. Elective residential admissions typically do.
